Immunization Requirements for Idaho Students

Idaho Statute 39-4801 requires students attending preschool and grades K-12 of any public, private, or parochial schools to meet the minimum immunization documentation requirements upon admission.

Parents and guardians must provide their child’s immunization record to school officials, unless an exemption is claimed (39-4801).

Immunization Exemptions

Idaho Statute 39-4801 allows a parent or guardian to claim an exemption from immunization requirements for their child for medical, religious, or other reasons

If an exemption is on file and there is an outbreak of the related disease, the exempted student may be excluded from school for the time deemed necessary by public health officials.

Required Documentation

As immunizations are updated, please submit a current copy of your child’s immunization record to the school nurse. If your child needs immunizations, please contact your healthcare provider or the Southwest District Health Department.
