Please complete an enrollment form, provide a certified birth certificate, current record of immunizations, and proof of residence.  The enrollment form may be picked up from the school. The enrollment form must be submitted to the school in your residential zone. If you wish to attend a school outside of your zone, please complete an Open Enrollment Form.
